Odometer disclosure statement form ODOM-65-3 with the traditional blue screen background.  An alternative version of this odom-65-3 form does exist that has a white background and does work better when the forms need to be scanned into computer systems.  The Truth in Mileage Act (TIMA) is a federal law that requires dealers selling motor vehicles to provide an odometer disclosure to the buyer at the time of sale or transfer of ownership. Buyers then must sign in acknowledgment of the mileage disclosure given.
